No. 28749 REQUEST FOR STATEMENT OF QUALIFICATIONS FOR ENGINEERING SERVICES Contingent upon sufficient funding, Flathead County Water and Sewer District #8, 103 Bear Trail, Whitefish, MT. 59937, is soliciting Statement of Qualifications for Consulting Services to assist with the preparation of design of approximately 2,300 lineal feet of 6 inch water main, appurtenances, fire hydrants and abandoning an existing well. Design services will include site surveys, hydraulic modeling, design, plans, specifications, MDEQ approval, Bid Documents, bidding and construction management. All services must conform to the current Montana Department of Environmental Quality Standards and American Recovery Act Plan (ARPA) requirements as adopted by Montana Department of Natural Resources. Generally the design will be for looping the existing water mains on Goat Trail to Fawn, Fawn to Elk Trail and Fawn to Deer Trail. The design has been approved by MDEQ for the water main from Goat Trail to Fawn. The plan will have to be updated for bidding purposes. Response to this request should include: Firm's name and address of corporate office. Location of managing office and contact information. Firm's history and areas of practice. Summary of experience, qualifications and location of staff to be assigned to the project. Summary of firms experience with providing services to water and sewer districts in Montana. Statement of experience with federally funded projects. Description of current work load. Tentative schedule for design, approvals and bidding of the project. Proposed work plan with milestone dates. Respondents will be evaluated on the following criteria: Overall quality of the proposal, 10%. Qualifications of professional staff assigned to the project, 30%. Prior performance in meeting schedule, budget and quality on previous projects, 15%. Availability to the District, 10%. Experience with water and sewer districts, 15%. Specific experience with similar projects. The response shall be limited to 10 pages, plus cover letter, cover page, table of contents and appendices with resumes, references and summary of similar projects. A page is a one sided single spaced 81/2" x 11" sheet of paper. The selection of the consultant may be based on the evaluation of the written response. At the discretion of the Board the District may conduct interviews of select firms. The District will select the most qualified firm based on the submitted information and references. Lundberg, a member of the Montana State Bar, as successor trustee, will sell at public auction to the highest bidder, the following described real property situated in Flathead County, Montana, and described as follows: Parcel A of Certificate of Survey No. 14286, located in the Southwest Quarter of the Southeast Quarter of Section 20, Township 28 North, Range 19 West, P.M.M., Flathead County, Montana. Parcel ID No.: 0604650 which has the address of 3255 Foothill Road, Kalispell, MT 59901.
